New Patient Paperwork
For your convenience, we recommend downloading and completing our New Patient Information Forms prior to your appointment. You can print and bring them to your appointment.
Our main goal will always be to treat our patients with respect while providing a warm and welcoming environment where they will receive the highest quality of dental care.
Dr. Howes is currently accepting new patients. Please call our office at 925.520.3191 to schedule an appointment. You may also request an appointment online by clicking here. Once you make an appointment at our office, that time will be reserved for you. If, for some reason, you are unable to keep your scheduled appointment time, call us with 24 hour notice to reschedule. If we receive no notice, a missed appointment fee will be charged.
What to Bring to Your First Appointment
Please arrive 15 minutes ahead of your scheduled appointment time to fill out necessary paperwork. If you would like to complete your paperwork beforehand, you may print and fill out the appropriate forms by downloading them here.
We offer our patients affordable monthly payment programs. You may qualify, call us to get the details at 925.520.3191 or let one of our team members know you are interested when you come in for your next appointment.
For more extensive treatment plans, we offer a reduction of the total fee if pre-payment is made at least 24 hours prior to the start of treatment. There will be a 5% discount for prepayment of patient portions, which exceed $400.
We accept most dental insurance plans. Please familiarize yourself with your insurance plan and pay close attention to your benefit maximums, deductibles, and covered percentages. Our staff will prepare all the necessary forms for your dental benefits. However, please remember that your specific policy is an agreement between you and your insurance company.
Please know that you are responsible for your total obligation should your insurance benefits result in less coverage than anticipated. We do not render our services on the basis that insurance companies will pay all of our fees. We base our treatment on what is best for you. Your benefits are contracted between your employer and your insurance company.
Payment is expected at the time services are rendered, unless other arrangements have been made. This includes any co-payments or deductibles (amount not covered by your insurance).
